On a personal level I can't talk about NHIF, one of my ailing elderly relatives was treated for a month as an inpatient at KNH and discharged without paying a single coin with the exception of KSHS 1500 at admission. Mark you, they were paying only @500 shillings for a month. The bill had accrued to hundreds of thousands, if it wasn't for NHIF?
