Everything, really.
Old oxygen plants fell into disrepair or became obsolete (case of KNH).
There aren’t nearly enough oxygen plants to generate as much as needed.
Systemic failure really. But we aren’t ready for that conversation. Yet.

Agreed, in the county I am in the hospital that had been designated as "the Covid19 response hospital" has had no oxygen from last November. Putting pressure on private hospitals in the county hq, and neighbouring counties.
