Someone told me they only had to fill a form. No testing whatsoever. This was months ago btw.

Someone told me they only had to fill a form. No testing whatsoever. This was months ago btw.

I've done that three times since May. Initially, they had some manual forms without MOH personnel to explain how to fill. Then they introduced a QR code that led to a form resembling Google docs that you were meant to voluntarily fill daily if you developed symptoms lol. It had tracking details too.
