Two things, poverty and a macho man culture. Hawa wasee huchimba choo na visima know if they insist the client provide excavation protection works (something as simple as a 900mm culvert), kazi inapotea.
But on the flip side kuna sites where the employer has provided all safety gear, lakini majamaa hawavai, hadi wanakuwa threatened na being ejected from site
But the only way for regulators to ensure safety inafwatwa is make such incidences very expensive to both employer and contractor
